How to Change Your Address, Name, or Taxpayer Identification Number (En Español)

  1. I am an individual. How can I notify the Department of Revenue of a change to my address, name, or social security number?
  2. I file a business tax return. How do I notify the Department of Revenue of a change to my address, name, or federal employer identification number?
  3. How should a town, village, city, county or other governmental unit notify the Department of Revenue of a change to the name, address, e-mail or telephone number for elected officials?

  1. I am an individual. How can I notify the Department of Revenue of a change to my address, name, or social security number?

    You can notify the Department of Revenue by:

    • Mailing Form P-706, Taxpayer Information Change Request Form, to the department. You may obtain this form by:
      • Downloading the form from the department's web site.
      • Requesting the form by calling our Forms Request Office at (608) 266-1961.
    • Including the necessary information in a letter to:

      Address Changes
      Wisconsin Department of Revenue
      P.O. Box 8949
      Madison, WI 53708-8949

    Include your name, tax account number or last four digits of your social security number, and address in all correspondence. Also include information specific to your request, including:

    • Your name
    • Your spouse's name
    • Change in your name or your spouse's name
    • Your old address
    • Your new address
    • County of your new residence
    • City, village, or township of your new residence
    • Incorrect social security number
    • Correct social security number

    Include your daytime telephone number in case we have questions. Include a copy of your social security card for a change in the social security number.

    If you are changing information for anyone other than yourself, Form A-222, Power of Attorney, must be provided.

  2. I file a business tax return. How do I notify the Department of Revenue of a change to my address, name, or federal employer identification number?

    A business can notify the Department of Revenue in one of the following ways:

    Address Change

    • Indicate the change on the next business tax return filed with the Department of Revenue.
    • Write to the Wisconsin Department of Revenue, P.O. Box 8902, Madison, WI 53708-8902. Include the name, federal identification number, old address and new address.

    • Submit an address change request through My Tax Account.

    Name Change & FEIN Change

    • Indicate the change on the next corporation or partnership return filed with the Department of Revenue.
    • Write to the Wisconsin Department of Revenue, P.O. Box 8908, Madison, WI 53708-8908. Include the old and new name, old and new federal identification number, and address.
    • My Tax Account users can submit requests to change their "doing business as" name or add their FEIN to their account if the department does not already have that number on record.

    Note: A corporation, limited liability company filing as a corporation or partnership, or limited partnership that is required to register with the Wisconsin Department of Financial Institutions (DFI) may change its name by writing to the Department of Financial Institutions, P.O. Box 7846, Madison, WI 53707-7846 or calling (608) 261-7577.

  3. How should a town, village, city, county or other governmental unit notify the Department of Revenue (DOR) of a change to the name, address, e-mail or telephone number for elected officials?

    You may mail, fax or e-mail the change(s) to DOR:

    • Mailing Address: Wisconsin Department of Revenue, Local Government Services Section 6-97, PO Box 8971, Madison, WI 53708-8971.
    • FAX Number: (608) 264-6887
    • E-mail

    Be sure to include:

    • The correct legal name of the governmental unit and the County and Municipality and/or Special District Code (if applicable).
    • Full name, title (position), mailing address, phone number and e-mail address of the new official.
    • Effective date.
    • Name of official being replaced.
