Form Information

  • Due date – November 15, 2023
  • Must e-file in MyDORGov
  • Provide municipalities in your county with the same apportionment values and referenda information reported to DOR on this form
  • These values auto-fill on the municipality's 2023 Statement of Taxes

Referenda Information (Section B of form)

You must enter the referendum information if both the following apply:

  • Your county approved a referendum for a temporary tax levy increase after December 31, 2014
  • The referendum increases your county's current year (2023) tax levy
