• File with the county treasurer

Form LC-220 – 2024 Manufactured/Mobile Home Municipal Permit – Wisconsin Lottery and Gaming Credit Application

• Owners of a manufactured/mobile home file this form to confirm they qualify for the lottery and gaming credit as of January 1, 2024
• File with the municipal treasurer
