The Wisconsin Department of Revenue, State and Local Finance Division (DOR) would like to remind you that Form LC-664: 2023-24 Manufactured/Mobile Home Lottery and Gaming Credit Report is due today, March 1, 2024.

Form Information

  • Municipal treasurer must e-file through MyDORGov
  • LC-664 – located on our website
  • Report the number of lottery credit claims your municipality issued and total amount your municipality paid only for manufactured and mobile homes with a monthly municipal permit fee collected
  • If this form is not filed timely, payment distributions will be delayed

Note: If you do not have manufactured/mobile homes receiving monthly municipal permit fees, you are not required to file this form.
