The Wisconsin Department of Revenue (DOR), State and Local Finance Division posted the Video Service Provider Report (Form SL-311) on our website.

General Information

• Due date – August 15, 2024
• Municipal clerk must e-file through MyDORGov
• Only municipalities that impose a video service provider fee under state law (sec. 66.0420(7), Wis. Stats.) must file
• Do not e-file Form SL-311 if your municipality does not impose a video service provider fee

Form Information

• The reported certification is used to confirm qualification of the 2025 video service provider aid for Wisconsin municipalities under state law (2019 WI Act 9)
• Each municipality that imposes a video service provider fee must report whether it imposed a fee in 2023
• Note: If your municipality no longer imposes the video service provider fee since it received video service provider aid and the mandated decrease of the video service provider fee decreased the fee you were imposing to 0%, you still must submit this form to receive your aid payment
