The Wisconsin Department of Revenue (DOR), State and Local Finance Division would like to remind you to file Form SL-311: Video Service Provider Report if you have not already done so

Important – to receive aid:

A municipality must submit this form to receive its video service provider aid payment even if the municipality is already receiving the aid

General Information

• Due date – August 15, 2023
• Municipal clerk must e-file through MyDORGov
• Only municipalities that impose a video service provider fee under state law (sec. 66.0420(7), Wis. Stats.) are required to file
• Do not e-file Form SL-311 if your municipality does not impose a video service provider fee

Form Information

• The reported certification is used to confirm qualification of the 2024 video service provider aid for Wisconsin municipalities under state law (2019 WI Act 9)
• Each municipality that imposes a video service provider fee must report whether it imposed a fee in 2022
• Note: If a municipality no longer imposes the video service provider fee since it received video service provider aid and the mandated decrease of the video service provider fee decreased the fee the municipality was imposing to 0%, the municipality must still submit this form to receive its aid payment
