The Wisconsin Department of Revenue (DOR) made the following forms available in My DOR Government Account. These forms must be electronically filed and are available on our
Government Online Filing web page.
County Forms
The County Treasurer submits both:
- Form PA-662: 2020 Lottery and Gaming and First Dollar Credit Report
- Report the number of lottery credit and first dollar credit claims issued and total amount paid for each municipality

- Form PC-501: 2020 Municipal Treasurer's Settlement
Municipal Forms
The Municipal Treasurer submits:
- Form LC-664: 2020-21 Manufactured/Mobile Home Lottery and Gaming Credit Report
- Report the number of lottery credit claims your municipality issued and total amount your municipality paid
only for manufactured and mobile homes with a monthly municipal permit fee collected
