The Wisconsin Department of Revenue (DOR) posted Form SL-311:
Video Service Provider Report to our
website for municipalities that impose a video service provider (VSP) fee under state law (sec.
66.0420(7), Wis. Stats.).
General Information

Important – this is a new form that municipalities
must file by the deadline. The only exception is if your municipality did not impose a VSP fee in 2019 and did not receive a 2020 VSP aid payment.

Due date – August 16, 2021
- Municipal clerk must electronically file (e-file) Form SL-311
- The reported certification is used to confirm qualification of the 2022 video service provider aid for Wisconsin municipalities under state law (2019 WI Act 9)
Form Information
- Each municipality that imposes a video service provider fee must report whether it imposed a fee in 2020
- Note: If your municipality no longer imposes the video service provider fee since it received video service provider aid and the mandated decrease of the video service provider fee decreased the fee you were imposing to zero percent, you still must submit this form to receive your aid payment
