Lottery and Gaming Credit Forms

• LC-300 – 2021 Late Lottery and Gaming Credit Application
• LC-310 – 2021 Late Buyer Lottery and Gaming Credit Application
• LC-320 – 2022 Late Manufactured/Mobile Home Municipal Fee for Lottery and Gaming Credit Application

Note: Property owners must submit a complete application, with the appropriate documentation, to DOR by October 1, 2022.
