The Wisconsin Department of Revenue (DOR) posted the below electronic forms on our Government Online Filing web page.

County Forms

County treasurer submits both forms:

• PA-662: 2021 Lottery and Gaming and First Dollar Credit Report
  • Report the number of lottery credit and first dollar credit claims issued and total amount paid for each municipality
  • Due date: March 1, 2022


• PC-501: 2021 Tax District Treasurer's Settlement
  • Review the form instructions
  • Counties must electronically file through MyDORGov or file transfer
  • Form no longer has state taxes line – updates include:
    • Section 1, Line 26 – now refers to Statement of Taxes, Section C instead of Section D
    • Section 3 – all line labels are updated; Line 28 (previously "state taxes") is now grayed out and labeled "reserved"
  • Due date: March 15, 2022

Municipal Form

Municipal treasurer submits this form:

• LC-664: 2021-22 Manufactured/Mobile Home Lottery and Gaming Credit Report
  • Report the number of lottery credit claims your municipality issued and total amount your municipality paid only for manufactured and mobile homes with a monthly municipal permit fee collected
  • Due date: March 1, 2022
