Form PA-662 – Change to Lottery and Gaming Credit Data File

The Wisconsin Department of Revenue (DOR) would like to inform you of an upcoming change that will impact your filing of Form PA-662: 2022 Lottery and Gaming and First Dollar Credit Report.

Updated instructions – Lottery and Gaming Credit post-certification county data files

You must submit your lottery and gaming credit post-certification county data files to DOR through MyDORGov by March 1, 2023. The files must be complete and follow the file layout provided by DOR, and must reconcile with Form PA-662.

Submitting your data file

  • Lottery and gaming credit post-certification county data files must be submitted in a .TXT file format
  • Attach a completed .TXT file to your Form PA-662 (located in MyDORGov) before submitting the form
  • Note: DOR must receive this file before we can process any lottery and gaming credit late claim applications

  • Available on our website in early February 2023
  • Will be located within MyDORGov
  • Filing deadline – March 1, 2023
