The Wisconsin Department of Revenue (DOR), State and Local Finance Division posted the 2024 Local Government Official Information forms on our website. As a county or municipal clerk, we require your assistance to ensure your local government's contact information is current and accurate.

Why is it important to update your contact information?

  • DOR updates our local government official contact information annually and provides other state agencies this data
  • Contact information's accuracy is important since DOR and state agencies use it to distribute payments, reports, and correspondence to local governments

Benefits – it's easier and more convenient to update

  • You can now easily update your own contact information from within MyDORGov, no need to email DOR staff with updates
  • Although this form is required annually, you can make updates throughout the year

Form Information

  • Due date – July 31, 2024
  • E-file the appropriate form through MyDORGov
  • When filing:
    1. Review each section and confirm listed information is accurate
    2. Update any incorrect information
    3. Acknowledge and submit the form to DOR
