Municipal Financial Report Form - Due May 15

The Wisconsin Department of Revenue (DOR) would like to remind you of the due dates for electronically filing (e-filing) the Municipal Financial Report (MFR) forms.

  • March 31, 2017 – populations under 2,500
  • May 1, 2017 – populations over 2,500
  • May 15, 2017 automatic extension – DOR grants an extension automatically for all municipalities and counties. You do not need to send a request for an extension.

The MFR forms are available on our website. Make sure to e-file the correct MFR form.

  • Form CT – for towns (without proprietary funds)
  • Form C – for cities and villages (and towns with proprietary funds)
  • Form A – for counties
