1. I am an individual and would like to update my information (address, name, social security number) with the Wisconsin Department of Revenue (DOR). How do I notify DOR of my updates?

   You can notify DOR by mail or fax. Steps include:

   • Complete the change request form
     • Form P-706​​, Taxpayer Information Change Request​​​
       
     • Call the Forms Request Office at (608) 266-1961​ to have the form mailed to you​

   When completing the form, include:

   1. Your name
   2. Tax account number (or last four digits of your social security number)
   3. Your spouse's name
   4. Change in your name or your spouse's name (if applicable)
   5. Your old address (if applicable)
   6. Your new address (including city, village or town, and county)
   7. Incorrect social security number (if applicable)
   8. Correct social security number
   
   

   Note:

   • Please include​ your daytime phone number in case we have questions
   • For changes to a social security number, include a copy of your social security card
   • If you are changing information for another party, you must also submit Form A-222, Power of Attorney

   • ​Submit an address change request through My Tax Account
   • Indicate the change on the next business tax return filed with the department

   • Write to the Wisconsin Department of Revenue, PO Box 8949, Madison, WI 53708-8949​​. Include the name, federal identification number, old address and new address.
   
   

   Name Change

   If you​ are only changing the business name on your account and your federal employer identification number (FEIN) did not change, provide the following information:
   

   • The current name on your account
   • Your tax account number or FEIN
   • Your new legal or "doing business as" name (DBA)
     
   • The date you want the change to take effect
   • A copy of the Articles of Amendment (required for legal name changes only)
   
   

   You may call our office at (608) 266-2776 to update your DBA. For legal name changes, fax the above​ information to (608) 327-0232, ATTN: Registration Unit, email DORRegistration@wisconsin.gov​, or mail it to the address below. We will mail you a new Wisconsin Business​ Tax Registration Certificate​ when we process your request.

FEIN Change​

If a new federal employer identification number (FEIN) is issued to you by the Internal Revenue Service (IRS), you must submit a new Wisconsin Business Tax Registration to DOR​.

Note: If you are required to submit a new Business Tax Registration, you should​ inactivate the previous tax accounts once the new tax accounts are established​.

3. How should a municipality, county or other governmental entity notify the Wisconsin Department of Revenue of a change to its elected official's contact information?

4. How do I update a governmental district's bank information for direct deposit of shared revenues (ex: First Dollar​ and Lottery and Gaming Credits)?
   • Contact the Local Government Services Bureau at lgs@wisconsin.gov to request Form DOA-6456: Authorization for Direct Deposit
   • Note:  As a local government, in Section 4 of this form, you must select "Yes" to indicate you receive payments from DOR's State and Local Finance Division
   • Mail completed forms to:

This form is only for government entities. Businesses must update their bank information through My Tax Account.
